Eligibility and scope

Applications identify the operator, locations, services and responsible professionals. Evidence of applicable permissions and insurance is required where relevant.

Assessment requirements

The assessment follows the delivery of the named services: who performs the work, how clients are informed, how consent is recorded and how equipment, hygiene and records are managed. Evidence should connect staff responsibilities and competence with the services included in the application.

Written procedures need to be supported by evidence of their use. Relevant records, interviews or observation may show how a concern is handled, how equipment checks are completed or how a decision to decline or refer a request is made. Remove unnecessary personal information from samples. The review also considers complaints and whether staff recognise the limits of their professional role.

Corrective action

Shortcomings are recorded against specific requirements. The applicant receives an opportunity to provide corrective evidence within a stated period. Serious unresolved risks prevent approval.

Appeals

Continuing responsibility

Recognition lasts 24 months with annual review. Changes affecting ownership, premises, services or professional responsibility must be notified. The public record shows the current status and scope.
